急！网上自己申请，家庭关系的54表问题？

说明里写的是amily register book (Hukou) and Form 54，用的是and，这个意思是说户口和54表都要上传？

根据这段文字来说，确实是两个都要的意思。
但网上查到，有前辈只提供户口本没有提供54表照样签出了。
所以，问下最近签出的朋友，都提供54表了吗？

我7月22日提交我和老公的，23日我出的，现在我老公还没出哈。我没有提交54表，只提交了户口本。

都需要提供。使馆列出清单也是两项都有的哈。

如果孩子不能自己签，可以由父母代签，签孩子名字，然后（父／母代签）
