急问1229表格有关问题

我准备暑假去澳洲旅游，前天提交了家庭2大1小的签证，我和老婆的已经出签。小孩的今天领事馆发email，要求提供1229，但是我理解1229是有不同行家庭成员才填写的吧？我要怎么填写1229？就填写第三页，其余不填写吗？

我们已经开始审理以上申请人的旅游签证申请，请补交以下材料:

• A copy of the natural parents’ ID card(s)
  

父母双方的身份证复印件

• Form 1229 or signed written authorisation by natural parents advising:
  

• Their agreement to permit their child to travel.
• The approximate date and duration of travel.
• Whether a single visit or multiple trips are permitted.
• Their contact information.
  

父母双方填写1229 号表格 或书面签名授权，并注明：

• 其同意允许其孩子赴澳
• 大致的赴澳日期及停留时间
• 允许其孩子一次还是多次赴澳
• 其具体联系方式（可联系到的电话或手机号码）

父母都同行，其实不用提交。当时在提示栏输入解释就可以。

这样你附上你们夫妻的出签信息，并写明情况，回邮过去。

感谢版主解答，我回邮附件父母身份证和两份出签资料，外加两人签名，说明一起同行前往，不知道签证官几天可以答复
